What’s Really in Your Bottle?

Why supplement quality deserves a closer look. Two bottles with the same nutrient name are not necessarily the same product.

Walk down a supplement aisle or scroll through one online and you will see a lot of promises.

Premium.

Natural.

Advanced.

Professional grade.

High potency.

Then there are the deals. Buy one, get one free. Buy one, get two free. Subscribe today and save 40%.

But none of those things answers one of the most important questions:

What am I actually getting in this bottle?

If you are taking something because you believe it may support your health, it is reasonable to want more information than what is printed across the front of the package.

Two bottles with the same nutrient name are not necessarily the same product.

What does supplement quality actually mean?

When we talk about quality, we are asking questions such as:

  • Does the product contain the ingredient listed on the label?
  • Does the amount reasonably match what the label says?
  • Has it been evaluated for relevant contaminants?
  • Was it manufactured using appropriate quality controls?
  • Is there independent verification of the product's contents and quality?

Then there is an entirely separate question:

Is this the right supplement, formulation, and amount for the person taking it?

Those are both important questions, but they are not the same question.

A supplement can be well manufactured and still be unnecessary or inappropriate for a particular person.

A nutrient may also have evidence supporting its use while a particular product containing that nutrient still has quality concerns.

Supplements are regulated, but differently from medications

You may have heard someone say dietary supplements are “unregulated.”

That is not accurate.

The FDA regulates dietary supplements, and manufacturers are required to follow federal requirements that include manufacturing and labeling standards.

There is, however, an important difference between supplements and medications.

In general, the FDA does not approve dietary supplements for safety and effectiveness before they reach consumers.

Manufacturers and distributors have the initial responsibility for making sure their supplements meet applicable requirements.

The FDA can inspect facilities, investigate complaints, review adverse event reports, examine imported products, monitor the marketplace, and take enforcement action when violations are identified.

FDA Good Manufacturing Practices are intended to help ensure the identity, purity, strength, and composition of dietary supplements.

Those protections matter.

They are not the same thing as having every finished supplement independently tested and approved before it appears on a store shelf.

What happens when researchers actually test the product?

A 2023 study published in JAMA provides a useful example.

Researchers purchased and analyzed 25 melatonin gummy products sold in the United States.

One contained no detectable melatonin.

Among the products in which melatonin was detected, the measured amount ranged from 74% to 347% of the amount stated on the label.

Overall, 22 of the 25 products were inaccurately labeled, using the study's definition, and only three contained melatonin within 10% of the declared amount.

That does not mean every melatonin supplement is inaccurate.

It certainly does not mean every dietary supplement has a labeling problem.

The researchers studied a small sample, analyzed only one sample from each brand, and looked specifically at gummy products.

But the study demonstrates something important:

The label and the laboratory result are not automatically the same thing.

That is one reason quality verification matters.

This is where independent testing can add another layer

The NIH Office of Dietary Supplements identifies several independent organizations that offer dietary supplement quality testing.

Depending on the organization and program, quality verification can provide information about whether a product:

  • Was properly manufactured
  • Contains the ingredients stated on the label
  • Contains the amounts declared
  • Meets specified limits for contaminants

Some certification programs also evaluate manufacturing facilities or conduct continuing surveillance.

But there is another important distinction.

Quality verification does not prove that a supplement works.

It also does not prove that a particular supplement is appropriate for you.

A quality seal can provide information about the product.

It does not replace the clinical question of whether that product belongs in your healthcare plan.

Don't confuse marketing with verification

A higher price does not prove quality.

A lower price does not prove poor quality.

A clinician only product is not automatically superior.

A bargain is not automatically inferior.

And words such as:

Premium

Natural

Professional grade

Advanced

High potency

do not, by themselves, tell you whether the finished product has undergone credible independent quality testing.

The same applies to beautiful packaging, celebrity endorsements, social media popularity, and thousands of online reviews.

Those things may influence what we buy.

They do not independently verify what is inside the bottle.

Change the first question you ask

Instead of beginning with:

“How much does it cost?”

or:

“How many milligrams does it have?”

start with:

“What gives me confidence that this product contains what it says it contains?”

Then ask:

“Is this actually the right formulation for what I am trying to accomplish?”

Because the next thing to understand is just as important:

Two bottles can contain the same nutrient and still be very different supplements.
